We have a range of healthcare programs delivered by an experienced registered nurse who will work with you to help you achieve the best possible outcomes. We use data science to understand current and future health risks and needs, and then actively help prevent, manage or treat disease risk.

Injury Support Program

Supporting injured workers in their recovery and return to health journey after a work place injury. This phone-based program will give you access to an experienced registered nurse to support with goal setting, health education and care coordination. Designed to complement the care provided by your GP, physiotherapist, or specialist, the program will include:

Personalised support via phone
Coordination and resources to help you better manage your health
Tailored Care Plan based on your health and recovery goals
Support to manage your recovery and return to work journey

Experiencing a workplace injury or illness can affect people in different ways. We will help you to be actively involved in your recovery and your return to health and work.

Hospital Support Program

This service is a phone-based program giving you access to an experienced registered nurse to help you navigate the health system and understand your options before, whilst and after coming home from hospital. Designed to accompany the usual care provided by your specialist or GP, the program will include:

Personalised support via phone
A health status assessment
Clinical decision making support
Support to close any knowledge or treatment gaps
Individualised Care Plan based on your health goals and priorities
Access to an experienced nurse to address chronic conditions and medical care coordination needs

This program is not available for pregnancy-related admissions.

Mental Health Hospital Support Program

The service is a phone-based program giving you access to an experienced mental healthcare nurse and tailored to meet your needs before, whilst and after coming home from an overnight mental health admission. Designed to accompany the usual care provided by your psychologist, psychiatrist or GP, the program will include:

A health status assessment
Clinical decision making support
Support to close any knowledge or treatment gaps
Individualised Care Plan based on your health goals and priorities
Access to an experienced nurse to address chronic conditions and medical care coordination needs

Supporting people with chronic conditions and a history of hospital admissions. This service is a phone-based program for people who can benefit from health education and care coordination. Designed to integrate with the usual care provided by your specialist or GP, the program will include:

Individualised Care Plan based on your health goals and priorities
Medical care coordination needs
Carer support

The Care Support Program provides access to an experienced registered nurse to help you better understand your chronic condition and achieve your health goals. Although program length varies according to individual needs, most people can expect to participate for 4 to 6 months.

Supporting people who are feeling overwhelmed, stressed, anxious or depressed and who have a history of overnight hospital admissions for mental health. This service is a phone-based program giving you access to an experienced mental health nurse and tailored to meet your needs. Designed to accompany the usual care provided by your psychologist, psychiatrist or GP, the program will include:

Education & health coaching
Support to close any knowledge or treatment gaps
Individualised Care Plan based on your health goals and priorities

Help connecting to community services and resources
Carer support

The Mental Health Care Support Program will help you better understand and manage your mental and emotional health. Although program length varies according to individual needs, most people can expect to participate for 4 to 8 months.

The COACH Program

Supporting people who have had, have, or are at high risk of having diabetes, cardiovascular disease, heart failure, and or chronic obstructive pulmonary disease. This service is a phone-based program that gives you access to your own personal ‘coach’ who are giving you access to qualified dietitians, pharmacists, or registered nurses. Designed to accompany the usual care provided by your health care team, the program will include:

Personalised support via phone
Education and health coaching based on guideline recommended target levels for your biomedical risk factors
Support to close any knowledge or treatment gaps
Support to help you understand and manage your medication
Individualised holistic plan based on your health goals and priorities
Follow up communication after each phone call

The COACH Program will empower and support you to prevent chronic disease development or progression. Although program length varies according to individual needs, most people can expect there to be 5 to 6 calls over a 6-month period.
