Experiencing an injury can affect people in different ways. Our Injury Support Program provides personalised support to improve the healthcare experiences of individuals during their recovery and return to health after an injury at work or a motor vehicle accident.
Designed to complement the care provided by a healthcare professional (GP, physiotherapist and or specialist), the program includes:
Our phone-based program gives individuals access to an experienced registered nurse to support them with goal setting, health education and care coordination. The program focuses on delivering outcomes for injured people across a number of areas.
These include:
- Connecting individuals with a nurse coordinator to support their road to recovery and to get things started on the right track.
- Providing personalised guidance and tools to empower individuals to take an active role in their recovery journey.
- Helping them to feel confident and in control as they navigate the road to regaining their health.
