What is the Injury Support Program?
A phone-based program that offers one-on-one guidance by connecting you with an experienced registered nurse to support you with goal setting, health education, and care coordination. This program is designed to complement the care provided by your GP, physiotherapist, or specialist.

What’s involved if I participate?
Once you’ve been invited to join the program and enrolled, you will have personalised phone support scheduled at a time that suits you. An injury nurse coordinator will assess your healthcare needs to provide you with a tailored care pan based on your health and recovery goals. You can receive advice and insight into treatment options available, being an available, positive partner on your road to recovery. The injury nurse coordinator will support you post-hospitalisation (should this occur) and help put services in place that may assist in reducing the risk of future hospitalisation and getting you on the right path to recovery.
